When corrections go astray:

The Books of The Times review on Feb. 26, about "The Fabric of the Cosmos" by Brian Greene, misidentified the location of notes where equations can be found. They are endnotes, at the end of the book, not footnotes, with the text. A request for this correction went astray last month at The Times.

"Went astray" sounds nicer than, "it just slipped our minds," but you get the point.
